B. Data Entry Formats used for the Field data collection in Maharashtra

Data for 686 data points were collected in formats designed by the Maharashtra team, without any technical
support or any latest survey software of data collection application software and with no backend programme
support. They designed the access to the forms by a Format that is placed below, giving the user the Data entry

forms to fill and submit using pdf, EXCEL and through QR Code, as per their ease of use.

Additionally, on revision of data points, they were separately collected, totalling 688.
